  • 摘要: 糖尿病是一种血糖失调导致代谢紊乱的慢性疾病, 胰岛素是临床糖尿病患者治疗的常用药物。不同类别的胰岛素因其体内作用机制和作用时间不同而表现出不同的临床疗效, 胰岛素的基础性长效释放和餐后控释一直是临床研究人员关注的热点。随着纳米技术的不断发展, 拥有优异载释药性能的新型纳米药物载体研发已成为解决临床靶向用药的重要契机, 尤其是金属基纳米载体对胰岛素的载释有望成为提升临床糖尿病患者疗效的有效路径。本文围绕胰岛素的可控释放, 简要介绍了临床胰岛素的基本类别和特征, 重点阐述了金属基纳米载体的合成及其对胰岛素的载释药性能。分别分析了金基、铁基、铜基、锌基等金属基纳米载体在胰岛素载释方面的性能, 通过具体参数对比介绍了不同金属基纳米载体对胰岛素载释的特点, 阐述了金属基纳米载体载释胰岛素的作用机制和典型特征, 同时探讨了载体尺寸、载体形貌及表面功能化对载体释放胰岛素的影响因素, 最后对现存问题进行思考和展望, 以期为临床糖尿病的治疗提供技术参考和治疗新思路。

     

    Abstract: Diabetes is a chronic disease caused by metabolic disorders due to abnormal blood glucose levels, and insulin is a commonly used drug in the treatment of diabetes. Different types of insulin exhibit different clinical efficacy due to their mechanisms and duration of action in the body, the basal long-acting release and postprandial controlled release of insulin have always been hot topics among clinical researchers. With the development of nanotechnology, the research and development of new nano-drug carriers with excellent drug release properties have become an important opportunity to solve clinical targeted medication, especially the loading of insulin by metal-based nanocarriers is expected to be an effective path to enhance the therapeutic efficacy in clinical diabetic patients. Herein, around the controllable release of insulin, the basic categories and characteristics of clinical insulin are briefly introduced, and the synthesis of metal-based nanocarriers and their drug release performance for insulin are emphasized. The performance of gold-based, iron-based, copper-based, and zinc-based metal-based nanocarriers in insulin loading are analyzed, the characteristics of different metal-based nanocarriers in insulin loading are introduced through specific comparative parameter, the mechanism and typical features of insulin loading elaborated by metal-based nanocarriers, and the influencing factors of carrier size, carrier morphology, and surface functionalization on the insulin release from carriers are also discussed. Finally, reflects on existing issues and provides insights into future directions, aiming to offer technical references and new ideas for the treatment of clinical diabetes.
