仁术健胃颗粒逆转慢性萎缩性胃炎伴胃癌前病变的药效及机制研究

The efficacy and mechanism of Renzhu Jianwei Granules in reversing chronic atrophic gastritis with premalignant lesions

  • 摘要: N-甲基-N'-硝基-N-亚硝基胍(N-methyl-N'-nitro-N-nitrosoguanidine, MNNG) 综合因素造模法诱导慢性萎缩性胃炎(chronic atrophic gastritis, CAG) 大鼠模型, 造模成功后灌胃给予仁术健胃颗粒, 动物实验经中国药科大学伦理委员会批准(批准号2022-09-029), 观察仁术健胃颗粒对CAG大鼠胃酸pH值、摄食量、体重, 血清中胃蛋白酶原(pepsinogen, PG)、胃泌素(gastrin, GAS)、胃动素(motilin, MTL) 及肿瘤坏死因子-α (tumor necrosis factor-α, TNF-α)、白细胞介素-1α (interleukin-1α, IL-1α)、白细胞介素-6 (interleukin-6, IL-6) 含量的影响; H&E染色、AB-PAS染色观察胃窦组织病理变化; TUNEL荧光染色观察胃黏膜上皮细胞凋亡情况; 免疫荧光染色检测胃黏膜PCNA、p53蛋白表达; 利用qRT-PCR检测胃组织中p53Mdm2Bcl-2BaxTnf-αIl-1αIl-6 mRNA表达; 利用Western blot检测胃组织中PTEN、Akt、NF-κB、p53、MDM2、Bcl-2、Bax蛋白表达, 利用免疫荧光双染技术检测胃组织中Bcl-2/Bax蛋白表达。结果显示, 仁术健胃颗粒可以显著降低大鼠胃酸pH值, 增加摄食量和体重, 降低血清中PGI/PGII、GAS、MTL、TNF-α、IL-1α、IL-6含量(P < 0.01); 升高胃窦组织中p53Bax mRNA表达水平(P < 0.05或P < 0.01), 降低胃组织中Mdm2Bcl-2Tnf-αIl-1αIl-6 mRNA表达水平(P < 0.01); 显著升高胃组织中PTEN、p53、Bax蛋白表达水平(P < 0.05或P < 0.01), 下调p-NF-κB、p-Akt、MDM2、Bcl-2蛋白表达(P < 0.05或P < 0.01), 综上, 仁术健胃颗粒通过干预PTEN/Akt/p53通路治疗CAG, 诱导细胞凋亡, 预防癌前病变。

     

    Abstract: Objective to study the efficacy and mechanism of Renzhu Jianwei Granules in treating chronic atrophic gastritis (CAG) with the precancerous lesions of gastric cancer, CAG rat models were induced using N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) comprehensive factor modeling method. After successful modeling, Renzhu Jianwei Granules were administered by gavage. Evaluating the efficacy of Renzhu Jianwei Granules by observing their effects on gastric acid pH, food intake, body weight, serum levels of pepsinogen (PG), gastrin (GAS), motilin (MTL), and inflammatory factors TNF-α, IL-1α, and IL-6 in CAG rats; Hematoxylin-Eosin (H&E) staining and Alcian blue-periodic acid Schiff (AB-PAS) staining were used to observe pathological changes in the gastric antral tissue; TUNEL fluorescence staining was used to observe apoptosis of gastric mucosal epithelial cells; immunofluorescence staining was used to detect the expression of PCNA and p53 proteins in the gastric mucosa; qRT-PCR was used to detect the mRNA expression of p53, Mdm2, Bcl-2, Bax, and Tnf-α, Il-1α, Il-6 in gastric tissue; Western blot was used to detect the protein expression of PTEN, Akt, NF-κB, p53, MDM2, Bcl-2, Bax in gastric tissue, and immunofluorescence staining technology was used to detect the protein expression of Bcl-2/Bax in gastric tissue. Animal experiments approved by the Ethics Committee of China Pharmaceutical University (approval No. 2022-09-029). The results show Renzhu Jianwei Granules significantly reduced the gastric acid pH in rats, increased food intake and body weight, and decreased the serum levels of PGI/PGⅡ, GAS, MTL, TNF-α, IL-1α, IL-6 (P < 0.01); increased the mRNA expression levels of p53, Bax in the gastric antral tissue (P < 0.05, P < 0.01), and decreased the mRNA expression levels of Mdm2, Bcl-2, Tnf-α, Il-1α, Il-6 in the gastric tissue (P < 0.01); significantly increased the protein expression levels of PTEN, p53, Bax in the gastric tissue (P < 0.05, P < 0.01), and downregulated the expression of p-NF-κB, p-Akt, MDM2, Bcl-2 proteins (P < 0.05, P < 0.01). To sum up, Renzhu Jianwei Granules treat CAG by intervening in the PTEN/Akt/p53 pathway, inducing cell apoptosis, and preventing precancerous lesions.
